Our coaches only take on serious, dedicated clients.
The Program requires dedication for the program to be effective. 

The survey below is intended for you to determine if you might benefit from the JVB Health & Wellness program.  All survey results are anonymous, we will not know who you are.

 

How it Works 

- Answer each question honestly.
- Select done when done with the survey.
- Your score will be calculated at the end of the survey.

Your Total Score

16 or more:
You are highly likely to be dedicated to the JVB health and wellness program and therefore benefit from working with a JVB health and wellness coach to meet your goals

14-15:  
You are likely to be dedicated to the JVB health and wellness program and therefore benefit from working with a JVB health and wellness coach to meet your goals

12-13:
You might benefit from working with a JVB health and wellness coach to meet your goals, but the JVB health and wellness program may require too much of a commitment for you at the present time

Less than 12:  
You are unlikely to commit to following the JVB health and wellness program and therefore are unlikely to benefit from working with a JVB health and wellness coach to meet your goals.  Take the survey again in the future when you are more likely to invest in your health

*Note:  People that have one or more of the following tend to struggle under the JVB Health and Wellness program until they work with a professional to resolve these issues (we call this Phase 1).  Once you have resolved these issues, the JVB health and wellness program can help you achieve your goals (we call this Phase 2).  If you have any of the following issues, we encourage you to speak to a professional to resolve these issues before starting the JVB Health and Wellness program. You (1) currently have any issues with eating and/or an eating disorder; (2) are currently suffering from severe depression; (3) are currently addicted to drugs, alcohol, and/or tobacco; (4) occasionally (or regularly) take pain and/or sleep prescription medication or other non-prescription illegal drugs; (5) if you struggle with a mood disorder (e.g., bipolar disorder); and/or (6) and/drink too much alcohol
